The best, quickest and safest way to order your repeat prescriptions is on-line by using the link on our homepage or alternatively by completing the tear off portion on your previous prescription.

Prescription FormPlease use the tear-off portion of your previous repeat prescription (the white side) to make your request.  Tick the appropriate boxes and send it to us or bring it along to the surgery or pop it into the prescription box.  Unless you have given specific instructions your prescription will be sent electronically to your nominate pharmacy as soon as the doctor authorises it. If you would like your prescription to be posted back to you, please enclose a stamped addressed envelope. Please allow 48 hours discounting Saturdays, Sundays and Bank Holidays before collection.  No prescriptions are dealt with at weekends.  

From time to time we may ask you to see your Doctor to review your medication and monitor your illness.

When attending hospital always take with you details of your current medication.

ORDERING OF MEDICATION – IMPORTANT INFORMATION

  • Please notify the surgery of any items included in your repeat order list that you no longer use and we will be able to remove them from your record.
  • Please do not tick items on your repeat request list until you are nearing the end of the last pack of that particular item.  This will avoid stockpiling of medication.  If possible, please try to arrange your repeat prescription requests so that they are ordered only once a month.
  • Please consider that medicines over-ordered on repeat prescriptions and stockpiled in your home can cause another patient’s treatment in the NHS to be delayed due to financial pressures.

The current prescription charge is £8.40 per item. You can apply for a Pre-payment Certificate by clicking on the link below and sending it off to the NHS Business Services Authority address on the form. 3 monthly prescription is £29.10 and 12 month prescription is £104.00
